Guidelines

1. If you can show academic success in your last semester at 91福利社 you are likely to be reinstated.
2. We encourage you to include documents that will support your reinstatement. The more documentation you attach, the more likely it is that you will be reinstated. Examples are death certificates, letters from physicians or therapists and letters of recommendation from counselors, educators or employers.
3. It is recommended, but not required, that your statement be reviewed by a  Counselor (Center for Educational and Retentions Counseling), indication that you have discussed your situation with him or her.

A campus committee will evaluate your Academic Dismissal Appeal Application.
